All host sites need to fill out this form by 5pm CT on December 8th.
A separate form must be submitted for each physical location.
This form is for host sites to provide information so that we can work with them on connecting the kiosk to the internet. For organizations that have a Model B kiosk, it also asks if they want the table to have wheels.
